If you miss either one of our Spring or Fall registrations, you can still join the team anytime during the year by calling 566-2430.
New team registrations will need (1) the Meet Sign Up Form, (2) the Swimmer Information Form, (3) a Medical Questionnaire/Release Form and (4) the USA Swimming Summer Registration Form (4 forms).
Swimmers coming from another team need the USA Transfer Form (5th form as required) in lieu of the Summer Registration Form.
Returning registrations need (1) the Meet Sign Up Form, (2) the USA Annual Swimming Registration Form, (3) the Swimmer Information Form, and (4) a Medical Questionnaire/Release Form. These last three forms may be omitted if you turned them in this past September and if there are no changes (1-4 forms, as applicable for changes).
IHSAA Wavier Form is for attending club competitions during the High School season. You may attend two meets with a waiver. You may not miss any High School meets to attend club meets.
